While a typical bruise (contusion) from minor trauma is harmless, extensive bruising from severe trauma can indicate life-threatening internal bleeding. It is not the bruise itself but the underlying vascular damage that poses danger.
Massive internal bleeding—such as from a car crash, crush injury, or ruptured organ—can lead to hypovolemic shock. This occurs when blood volume drops too low to perfuse vital organs, causing organ failure and death if not treated emergently.
Rarely, a deep bruise (hematoma) can become infected, especially if the skin is broken or immune function is compromised. Bacteria entering the bloodstream can cause sepsis, a systemic inflammatory response that is life-threatening.
Severe swelling within a closed muscle compartment (e.g., after a fracture or crush injury) can cut off blood flow, leading to nerve and muscle death. This is a surgical emergency.
Ordinary bruises heal on their own. Serious complications stem from the cause of the bruising, not the bruise itself.
